How to become a Paediatrician — your road, class by class

Class 8 to working life·15 student doubts answered ·Last checked 25 Oct 2026

The one-line route: Class 12 with Physics, Chemistry, Biology and English → NEET (UG) → MBBS with a 12-month internship → NEET-PG (or INI-CET) → 3-year MD Paediatrics or DNB → optional 3-year super-speciality such as D.M. Neonatology. [1] [2] [3] [4]

Pick your class and the road below starts where you stand.

00 The job

What the work really is

A paediatrician is a doctor for babies, children and teenagers. The specialist degree is MD (Paediatrics) or DNB after MBBS; NMC's PGMER 2023 also lists the 2-year Diploma in Child Health (D.C.H.). [3] [4]

A normal working day

  • Examine and treat sick children in OPD, wards and emergency.
  • Look after newborns, including premature babies in neonatal units.
  • Track growth, development and vaccination, and counsel parents.

Pay

Pay
LevelPay (official)
Government doctor via UPSC CMS 2026Pay Matrix Level 10 (₹56,100 – ₹1,77,500) + non-practising allowance [5]

Specialist and private paediatric salaries are not officially published; PG residents get a stipend at par with government colleges in the state.

Main routes

MBBS → NEET-PG → MD Paediatrics

NEET-PG is the single entrance for MD, MS and PG Diploma seats in government, private and deemed colleges. A broad-speciality MD takes 3 years. [3] [4]

DNB Paediatrics

NBEMS's post-MBBS DNB seats are also filled through the NEET-PG rank. [3]

INI-CET for AIIMS, JIPMER, PGIMER, NIMHANS

These institutes do not admit through NEET-PG — they hold INI-CET. [3]

Super-speciality

D.M. Neonatology, Paediatric Cardiology, Paediatric Critical Care, Paediatric Gastroenterology, Paediatric Nephrology, Paediatric Neurology and Paediatric Oncology take MD / DNB Paediatrics as the feeder degree. [4]

Time and cost

Time after Class 12
MBBS with 12-month internship, then a 3-year MD [2] [4]
NEET-PG 2026 fee
₹3,500 General / OBC / EWS · ₹2,500 SC / ST / PwBD [3]
NEET (UG) fee (India centre)
₹1,700 General · ₹1,600 EWS / OBC-NCL · ₹1,000 SC / ST / PwBD / third gender [1]

MBBS and PG tuition fees differ by state and college, so they are not printed here.

Hidden challenges

  • NEET-PG needs the 50th percentile (General / EWS) just to qualify, and qualifying does not by itself give a seat. [3]
  • You cannot sit NEET-PG until your 12-month internship ends by the bulletin's cut-off date (30 September 2026 for NEET-PG 2026). [3]
  • Paediatric Surgery is a different road: M.Ch. Paediatric Surgery takes MS / DNB General Surgery, not MD Paediatrics. [4]

Who should not choose it

  • You lose patience with crying children or anxious parents.
  • You cannot handle night duty in newborn and emergency units.
  • You want to start earning early — the road is MBBS plus at least 3 more years.
01 Your road, stage by stage

What to do at each stage — and the doubts on the way

  1. You are here Class 8 · Class 9

    Biology, Chemistry and patience

    • NEET (UG) has Physics 45, Chemistry 45 and Biology 90 questions — 180 in all, 720 marks. [1]

Doubt What does a paediatrician do?

Treats babies, children and teenagers. The specialist qualification is MD (Paediatrics) or DNB after MBBS; PGMER 2023 also lists a Diploma in Child Health (D.C.H.). [3] [4]

Next: Read the Routes list above.

Doubt How many years does it take to become a paediatrician?

MBBS with a compulsory rotating internship of at least 12 months, then a 3-year MD Paediatrics after NEET-PG. [2] [4]

Next: Plan for a long road and pace yourself.

  2. You are here Class 10

    Choose PCB with English

    • NMC allows NEET (UG) only after 10+2 with Physics, Chemistry, Biology / Biotechnology and English. [1]

Stream / subjects: Science with Biology (PCB) and English.

Doubt Which subjects do I need in Class 11 and 12?

Physics, Chemistry, Biology or Biotechnology, and English — the subjects NMC requires for NEET (UG). [1]

Next: Take PCB with English in Class 11.

  3. You are here Class 11 · Class 12

    NEET (UG) for MBBS

    • NEET (UG) is 180 questions for 720 marks in 3 hours, with +4 for a correct answer and −1 for a wrong one. [1]
    • Minimum age is 17 on or before 31 December of the exam year; there is no upper age limit. [1]

    Lower NEET (UG) fee

Doubt What percentile do I need in NEET (UG)?

The 50th percentile for General / EWS, the 40th for SC, ST and OBC and the 45th for PwBD (General) to be eligible for admission. [1]

Next: Aim far above the cut-off — seats go by rank.

Doubt Is there an age limit for NEET (UG)?

You must be 17 on or before 31 December of the exam year. There is no upper age limit. [1]

Next: Prepare without worrying about an age cap.

  4. You are here In college

    MBBS and internship

    • After the final MBBS exam you do a rotating internship of at least 12 months; interns get a stipend fixed by the authority that runs the college. [2]

    MBBS internship stipend

    Doubt Are MBBS interns paid?

Yes. Interns get a stipend fixed by the authority that runs the college during the rotating internship of at least 12 months. [2]

Next: Ask your college for its current rate.

Doubt When can I write NEET-PG?

Only after MBBS with your 12-month internship completed by the bulletin's cut-off — 30 September 2026 for NEET-PG 2026. Applications from candidates finishing later are rejected. [3]

Next: Track your internship end date against the bulletin.

Doubt Is NEET-PG the only way into MD Paediatrics?

For almost all seats, yes — All India Quota, state quota, private and deemed colleges and DNB. AIIMS, JIPMER, PGIMER and NIMHANS admit through INI-CET instead. [3]

Next: Prepare for both NEET-PG and INI-CET.

  5. You are here Graduated

    NEET-PG and MD Paediatrics

    • NEET-PG: 180 MCQs in 3 hours 30 minutes, computer-based; +4 correct, −1 wrong, in five timed sections you cannot revisit. [3]
    • Every MD / MS student does a compulsory 3-month District Residency Programme in district hospitals. [4]

    Lower NEET-PG feePG resident stipend

Doubt What is the NEET-PG cut-off percentile?

50th percentile for General / EWS, 45th for UR PwBD and 40th for SC, ST and OBC (including their PwBD) in NEET-PG 2026. [3]

Next: Remember seats go by rank, not just by qualifying.

Doubt Who fills the All India Quota PG seats?

The Medical Counselling Committee (MCC, DGHS) fills the 50% All India Quota, deemed and central university seats and all-India open DNB seats. State quota seats go through each state's counselling. [3]

Next: Register for both MCC and your state's counselling.

Doubt MD Paediatrics or Diploma in Child Health?

MD is 3 years; a PG diploma is 2 years. PGMER 2023 lists Paediatrics (D.C.H.) among recognised diplomas, but the D.M. super-specialities list MD / DNB Paediatrics as their feeder. [4]

Next: Prefer MD / DNB if you may super-specialise.

    Doubt Are PG residents paid?

Yes. PGMER 2023 says PG students in all institutions get a stipend at par with PG students of state or central government medical colleges in that state. [4]

Next: Ask your college for the current stipend.

  6. You are here Working / switching career

    Super-speciality and government jobs

    • D.M. courses (3 years) such as Neonatology and Paediatric Cardiology take MD / DNB Paediatrics as the feeder qualification. [4]

Doubt How do I become a neonatologist?

After MD / DNB Paediatrics, take the 3-year D.M. Neonatology — PGMER 2023 lists MD / DNB Paediatrics as its feeder qualification. [4]

Next: Prepare for the super-speciality entrance after MD.

Doubt Can a paediatrician become a paediatric surgeon?

Not directly. M.Ch. Paediatric Surgery takes MS / DNB General Surgery as the feeder, so surgeons and paediatricians follow different roads after MBBS. [4]

Next: Choose Surgery at NEET-PG if you want to operate on children.

Doubt What do government doctors earn?

Doctors recruited through UPSC CMS 2026 start at Pay Matrix Level 10 (₹56,100 – ₹1,77,500) plus non-practising allowance. [5]

Next: See the UPSC CMS guide.
